    Community Action Programs in Maricopa County are hosting walk-in events for utility assistance for low-income households- No Appointment is necessary at these events! If unable to attend one of these events, CALL the Community Action Program in your area (reverse side). If you have received utility assistance in the past 12 months, please call your CAP office to schedule an appointment as you may not be eligible at this time.

    Maricopa County residents living outside of Phoenix can attend these walk-in events.* Persons are seen on a first-come, first-served basis. Residents will be given a number in order of their place in line. A number is not a guarantee of an interview. Residents given numbers that morning may be interviewed and pre-screened to apply for UTILITY ASSISTANCE ONLY. An interview is not a guarantee of assistance. *Phoenix Residents may attend the event in Glendale

    You MUST bring with you the following:

    • U.S. birth certificate or U.S. passport or proof of current legal resident status for the applicant (Original or un-tampered copy)
    • Social Security Cards or verification of SS numbers for ALL household members
    • Current utility (energy) bills (SRP, APS, SWG only); M-Power customers should bring printout to show purchases for past 30 days, name of customer of record, and service address.
    • Proof of all income received by any member of households from any source, for 30 days up to and including the day you apply. (See reverse for income limits) Examples:
      • Employment - Check stubs for last 30 days from day of application
      • DES - DES case profile to show the following: family composition/FOOD STAMPS/CASH benefits
      • Child Support - child support print-out from court or child support agency to show 30 days income
      • Social Security/VA/Pension benefits - current award letters for present year
      • Unemployment benefits - Unemployment printout to show 30 days of payments, including day you apply.
      • IF NO income in 30 days - Proof of how you’ve been meeting basic needs (Food, rent/mortgage, utility payments, etc) since last receiving income, AND must provide proof of most recent income- i.e., last day worked, date paid and GROSS amount of final check on company letterhead.
